Requirements & Fees
In order to pull the right record, clerks in the office also need to know the following information: the full name of the person on the birth certificate (including maiden name), date of birth, city or town and county of birth, father’s and mother’s full names (including the mother’s maiden name), your relationship to the person on the birth certificate and the reason you’re requesting a birth record.
There is a 100 year waiting period before the general public can view the record. During this time, only the immediate family and the person on the birth record can view the record.
