Requirements, Limitations & Fees
Before you order a birth record, know that only the person on the birth record, the immediate family and the designated legal representative can view the birth certificate for the first 100 years after the birth record’s creation. To prove that you are able to access this information, you’ll be asked the following questions when you order your copy: the full name of the person on the birth certificate (including maiden name), date of birth, city or town and county of birth, father’s and mother’s full names (including the mother’s maiden name), your relationship to the person on the birth record and the reason you’re requesting a birth record. Lastly, you’ll need to include the following form:

The fastest way to get a current birth record is online at either

www.ExpressVitalRecords.com. For in-person or by mail requests, you may contact the Cleveland County Health Unit or the Arkansas Department of Health. All three request methods will issue the same birth record copy.
