They are issued at birth, and usually contain such vital information as the baby’s name, the baby’s birth weight, time of birth, the date of birth, city of birth, attending physician, the mother’s name and the father’s name.

 

Requirements, Limitations, & Fees

 

In Arkansas, it is against the law to release birth records less than 100 years old; during this time, only the person on the record, immediate family or the designated attorney handling legal matters can view the record.

 

A check or money order must be enclosed with the request; the first record is $12, and each additional record is $10.

 

You must also include this form with your request: http://www.healthy.arkansas.gov/programsServices/certificatesVitalRecords/Documents/vr-7_birth_certificate_application.pdf, and the following information: the full name of the person on the birth certificate (including maiden name), date of birth, city or town and county of birth, father’s and mother’s full names (including the mother’s maiden name), your relationship to the person on the birth record and the reason you’re requesting a birth record.
