Birth Record Limitations
Unlike some states, birth records are sealed for 100 years after creation, and during this time only the person on the record, immediate family or the designated attorney can view the record.
Birth Record Requirements
So that the clerks can pull the right record, you need to include: the full name of the person on the birth certificate (including maiden name), date of birth, city or town and county of birth, father’s and mother’s full names (including the mother’s maiden name), your relationship to the person on the birth record and the reason you’re requesting a birth record. Lastly, this form needs to be filled out and sent in with your request:
Birth Record Fees
Birth records cost $12 for the first copy (non-refundable) and $10 for each additional copy.
