Birth Record Requirements
Each birth record will cost $12 for the first copy (non-refundable) and $10 for each copy thereafter.
With your request, you’ll need the following information: the full name of the person on the birth certificate (including maiden name), date of birth, city or town and county of birth, father’s and mother’s full names (including the mother’s maiden name), your relationship to the person on the birth record, and the reason you’re requesting a birth record. This form must be included as well:
Birth Record Limitations
Unless you are the person on the record, immediate family, attorney of the family or person on the record, you must wait 100 years before you can view the information contained in an Arkansas birth record.
