Availability
If you want a death record with a cause of death on it, you will need to be a member of the immediate family (the spouse, parent, child, grandchild, or sibling). You can also provide documentation that you are acting on behalf of the person on the death record.
A death record that doesn’t have the cause of death is available to any member of the public. Additionally, a “cause of death” death record is also available to the general public 50 years after it was created.

You will need to make a photocopy of your picture ID, an enclose $5 for the first copy of a death record.
Death Record Copies from the State’s Health Department
There are several ways to order a death record: you can mail your request in, phone it in, fax it in, or place the request in person to the state’s office. The information is below:
