Jefferson County, Arkansas Vital Records
Vital records consist of your birth records, death records, marriage records and divorce records. Just because they are your vital records, it does not mean that anyone can view them. Marriage and divorce records are never public, and only the wife, husband, their lawyers and the immediate family may view the information. Birth records and death records are different. You must wait 100 & 50 years (respectively) to view the records if you are the general public. During this time period, only the person who is on the record, authorized attorneys and the immediate family can obtain a birth/death record.

Birth records cost $12 (also non-refundable) for the first copy that you request and $10 for additional copies that you request at that time. So the clerks can find the right record, you need to include this form:

Along with this form, you must include the following information: the full name of the person on the birth certificate (including maiden name), date of birth, city or town and county of birth, father’s and mother’s full names (including the mother’s maiden name), your relationship to the person on the birth record and the reason you’re requesting a birth record.

A coupon is a document issued from the Arkansas Department of Health which is the same as a marriage or divorce record. Enclose $10 and the following correct form:
