Vital records are composed of your birth records, death records, marriage records and divorce records.
 
Birth Records
 
Unless you are the immediate family or legal counsel of the party on record, you must wait 100 years before you can view a birth record in Arkansas.
 
The first birth record that you order is $12 (non-refundable), and fees for additional copies are $10 each. Along with payment, you will need to include this form: http://www.healthy.arkansas.gov/programsServices/certificatesVitalRecords/Documents/vr-7_birth_certificate_application.pdf. Lastly, you’ll need to include: the full name of the person on the birth certificate (including maiden name), date of birth, city or town and county of birth, father’s and mother’s full names (including the mother’s maiden name), your relationship to the person on the birth record, and the reason you’re requesting a birth record.
 
Death Records
 
Unless you are the deceased’s immediate family or an estate counsel, you must wait 50 years before you can view a death record.
 
The first death record that you order is $10 (non-refundable), and fees total $8 for each copy thereafter that you request. Each request will require this form: http://www.healthy.arkansas.gov/programsServices/certificatesVitalRecords/Documents/vr-8_death_certificate_application.pdf. Lastly, this information will need to be included: the full name of the deceased, the date of death, the county or city of death, the name of the funeral home (optional), your relationship to the deceased, and your reason for requesting the certificate.

Marriage and Divorce Records (Coupons)
 
The Arkansas Department of Health publishes marriage and divorce coupons, which are similar to records. Each coupon is $10 and you must include the correct form:
