Pulaski County, Arkansas Vital Records
When you view vital records, you are viewing a person’s birth records, death records, marriage records and divorce records.
Birth Records
Only the immediate family and legal counsel, can view the record during the first 100 years after creation in Arkansas.
You will need to pay a fee of $12 (non-refundable) for the first copy and $10 per additional copy of a birth record that you request. This form will also need to be included:
http://www.healthy.arkansas.gov/programsServices/certificatesVitalRecords/Documents/vr-7_birth_certificate_application.pdf, as well as this information: the full name of the person on the birth certificate (including maiden name), date of birth, city or town and county of birth, father’s and mother’s full names (including the mother’s maiden name), your relationship to the person on the birth record and the reason you’re requesting a birth record.
Death Records
Like birth records, there is a 50-year waiting period for death records; unless you are the deceased’s immediate family or an estate counsel, you will have to wait before you can view the information.

The Arkansas Department of Health issues coupons rather than records, and each coupon is $10; you must also include the proper form:
