Vital records are birth, death, marriage and divorce records.

 

Birth Records

 

Birth record requests will require this form:   http://www.healthy.arkansas.gov/programsServices/certificatesVitalRecords/Documents/vr-7_birth_certificate_application.pdf and the following information: full name of the person on the birth certificate (including maiden name), date of birth, city or town and county of birth, father’s and mother’s full names (including the mother’s maiden name), your relationship to the person on the birth record, and the reason you’re requesting a birth record.

 

Death Records

 

Death records will require this form: http://www.healthy.arkansas.gov/programsServices/certificatesVitalRecords/Documents/vr-8_death_certificate_application.pdf, and this information: the full name of the deceased, the date of death, the county or city of death, the name of the funeral home (optional), your relationship to the deceased, and your reason for requesting the certificate.

Marriage and Divorce Records

 

Marriage and divorce records are $10 when ordering from the Arkansas Department of Health.  You will also need to include the following correct form:
